Your First Visit
Our goal is to make you feel comfortable and at ease. We will discuss your case and evaluate the best course of action while taking the time to answer all of your questions and concerns.
What to Bring
Be clear as to why you are being referred to a surgeon so we may discuss your case in detail. Bring your insurance card, a form of ID, a list of your medications with doses, and a list of prior operations.
Patient Forms
To save time on the day of your visit, please download and fill out the patient forms provided (see “Patient Forms” tab under “Before Your Visit”) and bring them with you. That way you can arrive 5 minutes before your appointment instead of 20 to 30 minutes before.
Consultation
Do not expect that your operation will take place the same day of the consultation, depending on work load and type of intervention, we may or may not be able to perform the procedure right away.
