If you are unable to keep your appointment, please provide at least 48 hours notice so we may offer your appointment to another patient.
Office Policy
We have put in practice the following office policies to best serve you. Please let us know if you need additional information.
Office Hours
Our office hours are Monday through Friday, 8:30 a.m. to 5:00 p.m. If you have an emergency after hours or on weekends, your call will be answered by our answering service. They will notify the doctor on call of your urgent concern.
Appointments
For a consultation in which you or your primary care physician suspect you may need surgery, we will ask you to return for the procedure. This will allow us to schedule the time for the procedure and to contact your insurance for authorization if needed.
Patient Forms
If you need to have forms filled out for disability, insurance or other reasons, we will fill the first form out for you at no cost, any additional forms will be completed at $10.00 per form.
